How to Enroll

Below you will find the summary information for each of the plans available. For details on employee eligibility, please view the “Eligibility Requirements” located to the right. For rate details, please contact PSATS Trustees Insurance at 800-382-1268 for a copy of our rate sheets. 

When your township or authority is interested in enrolling, a resolution must be adopted and a Joinder Agreement must be completed. In addition, please complete the “Election of Participation” form (located to the right) and return it to our office. You will use this form to select the insurance plan(s) in which you wish to participate. 

Enrollment forms must be completed for each enrolling employee and must accompany the “Election of Participation” form. A waiver must be completed for each eligible employee that chooses to opt out of any of the benefits offered. Enrollment forms are located to the right and can be submitted three ways:

Depending on the product(s) chosen, there may be supplemental paperwork required, however, this will get the process officially started. We require the submission of the above paperwork no less than 30 days prior to the proposed effective date.

Medical

Below are five PPO (Preferred Provider Organization) plans available with the PSATS Trustees Medical Insurance services. The plans include a pharmacy benefit.  The PPO provider network for the medical insurance is the Aetna Open Choice PPO network.  With the Aetna network you will have access to 80,000 providers and all the 213 acute care hospitals in Pennsylvania.

Prescription Plan

Dental

The PSATS Trustees Insurance Fund offers you two valuable dental plans through Aetna Insurance. Both dental plans will be passive PPO, which means the benefits are the same for both in-network and out-of-networks services. Therefore, if your dentist is not in the network, the benefit will be the same and you will not see a decrease in coverage.

Life

Below are six Life Insurance Plans provided to townships through Unum Insurance. Each plan includes a matching Accidental Death and Dismemberment Benefit. There is no change in benefit unless you reach age 70. For active employees age 70 or over, the benefit will be 50 percent of the benefit that was in effect on the date before attainment of age 70.

Each plan includes an Accelerated Death Benefit option. (Under special circumstances, a portion of the life benefit is payable prior to death).

Vision/Hearing

Underwritten by National Vision Administrators (NVA), our Vision Insurance Plan will have you seeing the savings clearly. This plan can be offered on either an employer-paid or employee-paid basis and has no minimum requirements for the size of your group. With the large provider network in Pennsylvania, members can easily find a provider or even nominate a provider to the network.

As an additional bonus to the Vision Insurance Plan, the Trustees Insurance and Retirement Services introduced a Hearing Service Plan in 2018. The hearing service plan offers up to 60% off brand name hearing aids and provides access to a large network of professional audiologists and ENT physicians.

Disability

The following Short-Term Disability Plans are provided to townships through Unum Insurance. These plans provide a disability income benefit for up to 35 weeks of disability. Benefits begin on the first day if the disability is the result of an injury and the eighth day if the disability is the result of sickness (unless you are hospital confined due to sickness, then disability begins the first day.)

Townships can either select Plan 1, Plan 2, or Plan 3.

Will pay you a benefit of $248 per week in the event of a disability.
Plan 1 Summary

(Employee must work 30 hours per week.)
Will pay you a benefit of $375 per week in the event of a disability.

Will pay you 60% of earnings to a maximum of $600 per week of disability.

Note: Benefits are payable for non-occupational injuries or sicknesses.

Medicare Integrated

We offer two Medicare Integrated Plans with a Prescription Drug Benefit to our member townships. Click here to determine if your township is in a county that is served by the Aetna PPO network.
